In order to make the intake process more efficient, you can now fill out all of your intake documents online and sign all the policy documents online through the patient portal, which you can sign into once you are ready to begin the counseling process.
You can also download all the forms below, print, and fill them out with good old fashioned pen and ink.
You may also come early to your first appointment and complete the paperwork beforehand.
How Do I Pay For Counseling?
Payment is required at the end of all counseling sessions.
For your convenience, we accept Cash, Check, Visa, MasterCard, Discover, American Express, or Debit for all sessions.
Paul and the counselors accept some Major Insurance companies.
We are also Out of Network providers, which means you pay for the services up front and we can give you a monthly bill with a description of services, which you will then submit to your insurance company to obtain the reimbursement you qualify for.
You will be asked to complete a form with credit card information to pay for a missed appointment fee. This card will only be used after notification of fees to be charged.
Please understand that when you make an appointment, we are reserving that time for you. You will be charged a fee for missed appointments not cancelled more than 48 hours in advance. All cancellations must be done with a phone call or voice mail–Emails or Text Messages will not be accepted.
The missed or late cancellation appointment fee is $80 dollars for patients utilizing insurance. For Cash Paying payments, you will be charged at the full rate or your normal counseling visit–either $120/hr or at a sliding scale rate.

Please bring your insurance card and subscriber information to the first visit.
Please be aware of co-pay, deductibles and co-insurance fees for your individual policy. You will need to verify coverage with your provider before your first appointment. If you do not have verification, you will need to pay for the appointment.
For out of network, it is much simpler. Just be aware of your out of network benefits, come to your appointment, pay the fee, and we can give you a bill as requested, or monthly to submit to your insurance company for the benefits you qualify for.
We can provide a Superbill (Service Overview) to submit for out of network insurance.
Please check with your insurance provider and check your coverage carefully by asking the following questions:
How much does my plan cover for an out-of-network provider?
Do I have mental health benefits?
What is my deductible and has it been met?
How many sessions per calendar year does my plan cover?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?
